Arizona Wellness Group is dedicated to providing you with the highest quality, cost effective care. We accept all major credit cards, personal checks, cash and Health Savings accounts.
Our office treats many patients with severe allergies and sensitivities to environmental triggers; we ask that you respectfully refrain from wearing scented products such as perfume, cologne, fragrant lotion, body spray, dryer sheets, etc. when visiting our office.
Sensitive patients may experience adverse reactions to scented products including respiratory distress, fainting or balance problems. If this occurs, you may be asked to reschedule.
We strive to offer same day appointments when they are available. It is important that we know ahead of time if you plan to cancel or reschedule your appointment. Please call us at least 24 hours in advance to let us know.
In the event of a no-show or less than 24-hour cancellation or reschedule, a $50 charge will be added to your account that must be paid prior to your next visit. A second no-show or less than 24-hour cancellation or reschedule will be charged the full amount of the scheduled visit.
